Pathway Caring for Children is excited to welcome a dedicated and understanding Licensed Social Worker (LSW) to our team as a Foster Care Case Manager. We are looking for someone who can navigate the challenges of foster care with grace, patience, and kindness. Position: Foster Care Case Manager

Location: Northeast Ohio (Hybrid)

Type: Full-Time

Key Responsibilities:

• Case Management: Provide comprehensive case management for children in foster care, ensuring all needs are met and regulations are followed.
• Foster Family Support: Serve as a primary resource for foster families, providing them with guidance, resources, and emotional support.
• Assessments & Planning: Conduct home visits, assessments, and service planning to ensure each child's needs are properly addressed.
• Collaboration: Work closely with the legal system, schools, medical providers, and other agencies to advocate for the child's best interests.
• Compliance & Documentation: Ensure that all case notes, files, and reports are completed accurately and in a timely manner to meet legal and organizational requirements.

Qualifications:

• Licensed Social Worker (LSW) in the state of Ohio (Required).
• Bachelor's Degree in social work or a related field.
• Experience working with children and families, preferably in foster care or a related area.
• Strong communication, organizational, and time management skills.
• Ability to work independently and as part of a multidisciplinary team.
• Valid driver's license and access to a reliable vehicle for travel.
